GAPP provides medically necessary in-home nursing and personal care support for eligible children with complex medical conditions. GAPP is not a substitute for Medicaid; Medicaid eligibility must be established first.
GAPP approval is based on:
- active Medicaid eligibility
- medical necessity
- a complete and accurate application packet
GAPP services are reviewed by Alliant Health Solutions on behalf of the state.

Supplemental Security Income (SSI) for Medically Fragile Children
SSI is a federal program administered by the Social Security Administration that may provide monthly financial assistance to children with severe medical conditions.
Children may qualify when:
- the condition causes marked and severe functional limitations
- the condition is expected to last at least 12 months or result in death
- household income and resources meet SSI guidelines
SSI is separate from GAPP and not required for GAPP eligibility, but it can help stabilize long-term Medicaid access and offset caregiving-related financial strain.
